Forms

These forms are required for enrollment.

Applications

• Application for Admission: This is the application for admission form.

Pick-Up Authorization

• Pick-up Authorization Form: This authorization form identifies who is allowed to pick-up your child at school.

Medical History

• Medical History Form: Your child’s physician will need to complete this form within six months of enrolling at Prairie Moon.

Exemption from Immunization

• Exemption from Immunization Form: If your family has chosen not to vaccinate your child, this exemption form is required.

Emergency Contact

• Emergency Contact Form: Prairie Moon usually handles these forms. We print this front and back, on cardstock and cut into half.  These cards will need to be notarized.  Your local bank should be able to do it free of charge.  The school will need two copies per child.  One for the classroom and one for the office files.

Directory

• Consent for Release of Class Enrollment for School Directory Form:  The school directory is distributed to all Prairie Moon families, teachers and board members and is intended to facilitate communication between all members of the community.

For Early Childhood Only

Milk Substitution

• Milk Substitution Form:  Milk is provided to children in the early childhood classes. If you would prefer to supply your child with an alternative such as soymilk, almond milk, or rice milk please complete this form and communicate your wishes to your child’s teacher.

Sunscreen, Lip Balm Permission

• Sunscreen, Lip Balm Permission Form:  Teachers will, with your permission apply sunscreen, lip balm and moisturizer (provided by you) on your child.
